The Office of 学生 金融援助 now offers online forms that are entirely electronic!  These forms allow you to complete them online, sign electronically and submit them without the need to separately print, 邮件, 或者传真给他们.  The web browser Edge does not support this software.

学生 and Parent forms:

学生 and parent forms are forms that require the signature of 这两个 the 学生 and a parent before the form can be electronically submitted.

• Getting started – the 学生 clicks the yellow button on the top of the form.
• 学生 enters their university username and password.
• 学生 completes their portion of the form.
• 学生 clicks the yellow “Submit to Parent” button on the bottom of the form.
• Parent receives an e邮件 with a link to the form.
• Parent completes their portion of the form.
• Parent clicks the yellow “Parent Signature” button on the bottom of the form last page to sign electronically.
• Parent will be taken to AssureSign website to electronically sign the form with a computer mouse or stylus.  点击完成.
• 学生 receives an e邮件 with a link to sign the form; check your University 电子邮件 for this (possibly in your junk folder).
• 学生 signs with a computer mouse or stylus electronically and clicks “Finish”
• Once signed it will be submitted electronically to our office!
• 学生 and Parent will receive a final e邮件 with the option to download the completed document.

学生 only forms:

学生 only forms are forms that only require the signature of the 学生 before the form can be electronically submitted. There are two (2) variations of 学生 only forms depending on Federal Guidelines:

Electronic Signature 形式

• Getting started – click the yellow button on the top of the form.
• Enter your university username and password.
• 填写表格.
• Click the yellow “学生 Signature” button on the bottom of the form.
• 学生 will be taken to AssureSign website to electronically sign the form with a computer mouse or stylus.  点击完成.
• Once signed it will be submitted electronically to our office!
• 学生 will receive a final e邮件 with the option to download the completed document; check your University 电子邮件 for this.

Digital Signature 形式

• Getting started – click the yellow button on the top of the form.
• Enter your university username and password.
• 填写表格.
• Type your FULL NAME on the signature line
• Click the blue “Submit” button on the bottom of the form to send it to our office electronically.
• 学生 will receive a final e邮件 with the option to download the completed document; check your University 电子邮件 for this.

形式 will be checked for accuracy before you can sign it.  Allow 2-3 business days for your MyISU Portal to reflect the submitted document. If any additional information is required, an e邮件 will be sent to the 学生s University e邮件 address.
